My name is Angela Forman, and I joined the team here at Spurlock Spine Centre in February 2007. I graduated from Morehead State University with a Bachelor's degree and I am currently working on my Master's.  My duties here include registering new and established patients, and many other administrative tasks. 

As a newlywed, I am enjoying married life and the time spent with my wonderful step-daughter.  When not working, I enjoy watching Nascar races, and spending time with family.

My name is Danielle Story and I am a massage therapist here at Spurlock Spine Centre.

My name is Shelley Knipp and I am a Chiropractic Assistant at Spurlock Spine Centre.  I joined the team in May 2008 and thoroughly enjoy working with our patients.  Seeing their progress from day to day makes for a truly fulfilling career. 

When not working I enjoy spending time with my husband and fourteen year old daughter.  I also volunteer with my local fire department and have been a certified fire fighter and emergency medical technician for nearly ten years.
